CRM offers a collaborative and client-central approach to case management for people living with life-changing neurological injuries and conditions.

What is Case Management?

Case Management is the management of services provided to people who have gone through a life-changing injury.

It is a process that is overseen by a Case Manager who will work alongside legal teams and other healthcare professionals to assess, plan, implement, monitor and evaluate the care and support services that a person living with a brain or spinal cord injury may require.

The fundamental goal of case management is to provide a holistic care plan to meet a person’s health, social care, educational and employment needs.

Our approach

Our case managers are experienced professionals in assessing the needs not just of a client, but their families and wider support specialists, too. Working with neuro physiotherapists, Speech and Language Therapy (SaLT) teams, occupational therapists and neuropsychologists, we develop tailored support packages that focus on high quality of life, and expert quality of care.

Assessment

We’ll start by gathering information from the person living with the injury, as well as their family, loved ones and relevant specialists. Our case managers will thoroughly assess the person’s cognitive, social, emotional and physical needs, building a well-rounded picture of their life, relationships, finances, safety and independence.

Planning

Once we have a deep understanding of the person’s needs and circumstances, we’ll set out a tailored plan to address guidance, support and rehabilitation goals. Each decision we make within this plan will be made in conjunction with the opinions of therapy experts. At this stage, we will also begin to recruit specialist carers tailored specifically to the care plan, in addition to addressing any advocacy issues.

Implementation

The next step is the most important: getting the care started. Throughout implementation, our case managers regularly review plans, monitor progress and maintain records that inform any changes or improvements that may be needed. Our support workers are handpicked to meet the individual personalities and needs of each client, following our duty of continuity of care to the people we support. This commitment is further backed up with a 24 hour on-call system which is available every day of the year.

Case management is a long-term service that requires continued input which may include support for everyday care, finding or adapting accommodation, hospital or therapy appointments, social outings, education or employment, and holiday or event organising.
